Grant Reporting

We request that all our grantees file a final report within sixty days of the end of the grant period specified in the original grant application document, or as amended. The Final Report Form can be found on our Web site. Organizations with late reports will not be considered for future funding until all overdue reports are received. For multi-year grants, we request that our grantees submit on each annual anniversary date an Interim Report which summarizes the progress made on each of the goals and objectives contained in the Grant Application. Click here to download the Interim Report.
